Abstract

Objective: This retrospective study was designed to evaluate the efficacy and safety of doxycycline in patients with a history of urinary tract infection (UTI) who received a prolonged period of therapy with doxycycline.

Objective: The study included 724 adult patients with a history of UTI who had received an antibiotic regimen with doxycycline for at least one year. The patients were randomly assigned to receive a 1:1, 3 mg daily doxycycline regimen followed by a 7-day course of oral doxycycline (400 mg). The primary outcome was the overall incidence of adverse events (AEs) during the study period.

Method: The study was conducted in the Department of Obstetrics and Gynecology of the University of Graz in Austria. The patients were included in the study if they had a UTI that lasted longer than 5 days or had no signs of infection, had an active history of UTI, were free of any other chronic disease or chronic illness, and had no history of other chronic illnesses such as diabetes mellitus, high cholesterol, hypertension, hypertension or smoking. The patients were followed for a mean period of 28.8 ± 17.4 months after the start of the study.

Patients were randomly assigned to receive the doxycycline regimen with oral doxycycline 400 mg for 7 days or with oral doxycycline 12.5 mg for 6 days (n = 6).

Main outcome measures were the incidence of AEs during the study period, and the percentage of the patients who discontinued treatment. The data were analyzed by using statistical package JASP v9.

Results: The mean age of the study patients was 39.9 ± 6.5 years. Overall, there were more AEs (14.8%) during the study period and an AEs (6.5%) during the 6-day course of treatment than during the 7-day course (2.3%). Among the patients who were free of other chronic diseases, the overall incidence of AEs was higher during the study period (18.8%) than during the 7-day course (3.7%). Among the patients who were free of other chronic illnesses, the overall incidence of AEs was higher during the study period (27.5%) than during the 7-day course (3.9%). The AEs most commonly recorded during the study period were those for diarrhea (2.3%), and nausea (2.8%), with a higher incidence in the doxycycline group compared with the patients on the 7-day course (2.7%) (P <.001).

Conclusion: The results indicate that doxycycline is effective for a period of 7 days in the treatment of patients with UTI who received a prolonged period of antibiotic treatment.
